Job Description:
Our client is seeking a skilled and dedicated Community Outreach Nurse to deliver high-quality nursing care and health education within the community. This role involves providing support and care to individuals in their homes, community centres, schools, and other non-clinical settings, with a focus on health promotion, disease prevention, and management of long-term conditions.

As a Community Outreach Nurse, you will play a key role in reducing health inequities and improving health outcomes for diverse populations. You will be working with a variety of communities, including Māori, Pasifika, and refugee groups, to ensure equitable access to healthcare services and support individuals in leading healthier lives.

Key Responsibilities:

  • Deliver nursing services to individuals and families in their homes or community settings, focusing on chronic disease management, health assessments, wound care, and post-hospital discharge support.
  • Provide health education and promote wellness by offering advice on a range of health issues such as nutrition, smoking cessation, mental health, immunisation, and preventative care.
  • Collaborate with other health professionals, social workers, and community organisations to coordinate care and ensure holistic support for patients.
  • Conduct home visits, school health screenings, and community health education events as part of outreach activities.
  • Raise awareness and promote public health campaigns aimed at preventing disease and encouraging healthier lifestyle choices.
  • Maintain accurate and up-to-date patient records in line with New Zealand health regulations and organisational policies.
  • Act as an advocate for patients, assisting with navigating the healthcare system and connecting individuals to additional support services as needed.
  • Participate in and contribute to team meetings, case conferences, and continuous professional development.

Essential Qualifications and Skills:

  • Registered Nurse with a valid Nursing Council of New Zealand (NCNZ) registration and a current APC (Annual Practicing Certificate).
  • Proven nursing experience, with a preference for candidates with a background in community nursing, public health, or chronic disease management.
  • Excellent clinical assessment and critical thinking skills.
  • Strong communication and interpersonal skills, with the ability to engage and build trust with individuals from diverse cultural backgrounds.
  • Ability to work autonomously and as part of a collaborative healthcare team.
  • A strong commitment to promoting health equity and improving health outcomes for all communities.
  • Full New Zealand driver's licence and access to a vehicle for home visits.

Desirable:

  • Experience in Māori and Pasifika health or working with diverse and underserved populations.
  • Training in health promotion or public health nursing.
  • Experience with mental health or addictions care.
  • Basic Life Support (BLS) certification.
